Assurity will now accept electronic signatures through DocuSign and Adobe Sign for the following post-issue forms and requests listed below. The Certificate of Completion DocuSign page or Final Audit Report Adobe Acrobat Sign page must also be provided along with the appropriate form(s).
- Policyholder Service Applications available through AssureLINK. State-specific applications for conversions, guaranteed insurability, policy change and reinstatement.
- Automatic Bank Payment for drafting premiums and/or loan repayments from a customer’s bank account. Be sure to use the post-issue request Form 18-051-05055.
- Dividend Option Changes (Form 18-657-01155) to change the annual dividend on a traditional life policy.
- Payment Option Changes (Form 18-656-05055)
- Servicing Agent Change Request (Form 18-696-05055)
While these are currently the only post-issue service forms we accept with electronic signatures, don’t forget that address changes and distribution requests like loans, withdrawals and surrenders are available on the MyAssurity customer portal for policyowners to access directly. We hope this new option will provide you more flexibility and help speed business.
