In an effort to streamline documents across departments, we are excited to announce that the following forms have been updated:
• Certification of Trust Agreement – Form number L-3172A
• Certification of Business Signing Authority – Form number O-2927
The new version of the forms have a revision date of 6-21 and replace any prior versions. Forms Factory has already been updated and SimpleSubmit® will be updated in the near future. The prior version of both forms will no longer be accepted as of January 1, 2022. |
